ABSTRACT

Severe aplastic anemia II (SAA-II) progresses from non-severe aplastic anemia (NSAA). The unavailability of efficacious treatment has prompted the need for haploidentical bone marrow transplantation (haplo-BMT) in patients lacking a human leukocyte antigen (HLA)-matched donor. This study aimed to investigate the efficacy of haplo-BMT for patients with SAA-II. Twenty-two patients were included and followed up, and FLU/BU/CY/ATG was used as conditioning regimen. Among these patients, 21 were successfully engrafted, 19 of whom survived after haplo-BMT. Four patients experienced grade II-IV aGvHD, including two with grade III-IV aGvHD. Six patients experienced chronic GvHD, among whom four were mild and two were moderate. Twelve patients experienced infections during BMT. One was diagnosed with post-transplant lymphoproliferative disorder and one with probable EBV disease, and both recovered after rituximab infusion. Haplo-BMT achieved 3-year overall survival and disease-free survival rate of 86.4% ± 0.73% after a median follow-up of 42 months, indicating its effectiveness as a salvage therapy. These promising outcomes may support haplo-BMT as an alternative treatment strategy for patients with SAA-II lacking HLA-matched donors.

ABSTRACT

By removing a part of the structure, the tooth preparation provides restorative space, bonding surface, and finish line for various restorations on abutment. Preparation technique plays critical role in achieving the optimal result of tooth preparation. With successful application of microscope in endodontics for >30 years, there is a full expectation of microscopic dentistry. However, as relatively little progress has been made in the application of microscopic dentistry in prosthodontics, the following assumptions have been proposed: Is it suitable to choose the tooth preparation technique under the naked eye in the microscopic vision? Is there a more accurate preparation technology intended for the microscope? To obtain long-term stable therapeutic effects, is it much easier to achieve maximum tooth preservation and retinal protection and maintain periodontal tissue and oral function health under microscopic vision? Whether the microscopic prosthodontics is a gimmick or a breakthrough in obtaining an ideal tooth preparation should be resolved in microscopic tooth preparation. This article attempts to illustrate the concept, core elements, and indications of microscopic minimally invasive tooth preparation, physiological basis of dental pulp, periodontium and functions involved in tool preparation, position ergonomics and visual basis for dentists, comparison of tooth preparation by naked eyes and a microscope, and comparison of different designs of microscopic minimally invasive tooth preparation techniques. Furthermore, a clinical protocol for microscopic minimally invasive tooth preparation based on target restorative space guide plate has been put forward and new insights on the quantity and shape of microscopic minimally invasive tooth preparation has been provided.

ABSTRACT

To compare the microleakage of fiber post and resin-core system fabricated by different methods.The roots of 36 recently extracted single-rooted mandibular premolars were undergone endodontic treatment.Fiber posts and Paracore flowable resin composite were used for fabricating post-core restorations.Microleakage was examined by dye penetration method.The microleakage value was significantly higher in Direct-Mold-Cement-Method group(4.94 ± 1.71) mm compared to Direct-Mold-Secondary-Cement-Method group(0.91 ±0.33) mm and Indirect-Mold-Secondary-Cement-Method group (0.87 ± 0.27) mm (P < 0.05).

ABSTRACT

General dentistry is an important part of the dental medicine and general dentists and general dentistry form the basis of clinical dental medicine. China's general dentistry has a long history, which started as an independent specialist in the 1990s. At present, the Chinese general dental medicine has received more and more attention as an independent profession. General dental medical model has been rapidly developed in the general hospital department of dentistry, private practice and community dentistry institutions, dental specialist hospitals and so on. In this paper, we will review the rise and development of China's general dentistry, and report its theoretical characteristics, institutional framework, academic progress, member development report, and look forward to its development in the future.

ABSTRACT

<p><b>OBJECTIVE</b>To explore the effect of glimepiride on the glucose uptake as well as glucose transporter (GLUT)-1 and GLUT-3 expression levels of rat mandibular osteoblasts in hyperglycemia.</p><p><b>METHODS</b>Primary osteoblasts were isolated and cultured. Then, the cells were placed in an osteogenic medium containing two glucose concentrations (5.5 and 16.5 mmol X L(-1)), with or without glimepiride (10 micromol x L(-1)). Glucose uptake was determined by employing 18F-deoxyglucose (18F-FDG) in the cells, and GLUT-1 and GLUT-3 expression levels were evaluated by Western blot analysis.</p><p><b>RESULTS</b>Glucose at 16.5 mmol x L(-1) significantly inhibited 18F-FDG uptake and downregulated GLUT-3 protein expression in osteoblasts. Hyperglycemia increased GLUT-1 protein expression. Glimepiride significantly increased glucose uptake and upregulated GLUT-1 and GLUT-3.</p><p><b>CONCLUSION</b>Glimepiride enhance the glucose transporter in rat osteoblasts at two different glucose concentrations.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the effects of orthodontic treatment of skeletal crossbite adults with mandibular deviation.</p><p><b>METHODS</b>Eighteen skeletal Class III adult patients with borderline skeletal crossbite and mandibular deviation were selected (5 males, 13 females). The mean age was 25 years. All cases were treated with straight-wire appliance in upper arch and occlusal plate in the lower arch.Elastics were applied to correct mandibular deviation. Cephalometric analysis was carried out before and after treatment. Paired t-test was performed.</p><p><b>RESULTS</b>After treatment, the anterior crossbite was corrected and Class I molar and canine relationships were achieved. The inclination of upper incisors was increased significantly.</p><p><b>CONCLUSIONS</b>Straight-wire appliance combined with occlusal plate was effective on correcting adult patients with mild, moderate skeletal crossbite and mandibular deviation.</p>

ABSTRACT

<p><b>BACKGROUND</b>The pain caused by orthodontic treatment has been considered as tough problems in orthodontic practice. Danggui-shaoyao-san (DSS) is a traditional Chinese medicine (TCM) prescription which has long been used for pain treatment and possesses antioxidative, cognitive enhancing and antidepressant effects. We raise the hypothesis that DSS exerts analgesic effect for orthodontic pain via inhibiting the activations of neuron and microglia.</p><p><b>METHODS</b>DSS was given twice a day from day 5 prior to experimental tooth movement (ETM). Directed face grooming and vacuous chewing movements (VCM) were evaluated. Immunofluorescent histochemistry and Western blot analysis were used to quantify the Iba-1 (microglia activation) and Fos (neuronal activation) expression levels in the trigeminal spinal nucleus caudalis (Vc).</p><p><b>RESULTS</b>ETM significantly increased directed face grooming and VCM which reached the peak at post-operative day (POD) 1 and gradually decreased to the baseline at POD 7. However, a drastic peak increase of Fos expression in Vc was observed at 4 hours and gradually decreased to baseline at POD 7; while the increased Iba-1 level reached the peak at POD 1 and gradually decreased to baseline at POD 7. Furthermore, pre-treatment with DSS significantly attenuated the ETM induced directed face grooming and VCM as well as the Fos and Iba-1 levels at POD 1.</p><p><b>CONCLUSION</b>Treatment with DSS had significant analgesic effects on ETM-induced pain, which was accompanied with inhibition of both neuronal and microglial activation.</p>

ABSTRACT

<p><b>OBJECTIVE</b>We used functional magnetic resonance imaging (fMRI) to explore the effects of noxious coldness and non-noxious warmth on the magnitude of cerebral cortex activation during intraoral stimulation with water.</p><p><b>METHODS</b>Six male and female subjects were subjected to whole-brain fMRI during the phasic delivery of non-noxious hot (23 °C) and no- xious cold (4 °C) water intraoral stimulation. A block-design blood oxygenation level-dependent fMRI scan covering the entire brain was also carried out.</p><p><b>RESULTS</b>The activated cortical areas were as follows: left pre-/post-central gyrus, insula/operculum, anterior cingulate cortex (ACC), orbital frontal cortex (OFC), midbrain red nucleus, and thalamus. The activated cortical areas under cold condition were as follows: left occipital lobe, premotor cortex/Brodmann area (BA) 6, right motor language area BA44, lingual gyrus, parietal lobule (BA7, 40), and primary somatosensory cortex S I. Comparisons of the regional cerebral blood flow response magnitude were made among stereotactically concordant brain regions that showed significant responses under the two conditions of this study. Compared with non-noxious warmth, more regions were activated in noxious coldness, and the magnitude of activation in areas produced after non-noxious warm stimulation significantly increased. However, ACC only significantly increased the magnitude of activation under noxious coldness stimulation.</p><p><b>CONCLUSION</b>Results suggested that a similar network of regions was activated common to the perception of pain and no-pain produced by either non-noxious warmth or noxious coldness stimulation. Non-noxious warmth also activated more brain regions and significantly increased the response magnitude of cerebral-cortex activation compared with noxious coldness. Noxious coldness stimulation further significantly increased the magnitude of activation in ACC areas compared with noxious warmth.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To establish a rapid and sensitive high-performance liquid chromatography (HPLC) method for detecting pazufloxacin concentrations in the saliva, gingival crevicular fluid and serum of healthy adults.</p><p><b>METHODS</b>Samples of saliva, gingival crevicular fluid and serum were obtained from healthy adults receiving intravenous infusion of pazufloxacin. The concentrations of pazufloxacin in the samples were quantified by HPLC equipped with a reversed-phase column (Agilent Zorbax SB-C18 5 µm, 250 mm×4.6 mm). The mobile phase for pazufloxacin was a mixture of acetonitrile and 0.5% phosphoric acid containing 1% triethylamine (155:850), and 20 µl of the resulting solution was injected into the HPLC system at a flow rate of 1.0 ml/min. The detection wavelength was set at 245 nm. The samples were first deproteinized by precipitation with methanol followed by supernatant drying; the residue was reconstituted with the mobile phase and centrifuged, and the supernatants were directly injected into the HPLC system.</p><p><b>RESULTS</b>Pazufloxacin in the samples were totally separated without interference by any endogenous substances. The calibration curves showed a good linear regression (r>0.999). The detection limit was 10 ng/ml with within-day and between-day coefficients of variation performance all below 5% and recovery rates all above 91%.</p><p><b>CONCLUSION</b>HPLC is both sensitive and selective for quantification of pazufloxacin in saliva, gingival crevicular fluid and serum.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the influence of background color on veneered In-Ceram and Cercon dental ceramics including Vita In-Ceram Electroformed Alumina (AL2), Electroformed Zirconia (Z21), Cercon base color Zirconia and Cercon base Zirconia.</p><p><b>METHODS</b>The colorimetric values of all the materials on noble metal, Ni-Cr alloy, silver amalgam and resin background were measured with a spectrocolorimeter, and the color difference among the groups was calculated.</p><p><b>RESULTS</b>After veneered, the color difference of Z21 Electroformed Zirconia was more distinct against the background of silver amalgam than against the backgrounds of noble metal and resin, but the color differences of the 4 veneered dental ceramics against different backgrounds were unidentifiable by human eyes (δE<1.5).</p><p><b>CONCLUSION</b>After veneered, AL2 electroformed alumina, Z21 electroformed zirconia, Cercon base Zirconia and Cercon base color Zirconia are less influenced by the background color, and covering the background color is therefore not necessary.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To explore the expression of glucose transporter (GLUT)-1 and insulin receptor (IR) alpha1 in osteoblast obtained from diabetic rats' mandibles.</p><p><b>METHODS</b>The expression of GLUT-1 and IR alpha1 of diabetic and control groups were measured by reverse transcription polymerase chain reaction, Western blot and immunohistochemistry stain.</p><p><b>RESULTS</b>The mRNA expressions of GLUT-1 and IR alpha1 of diabetic group were significantly higher than control group (P<0.05). The protein expressions of GLUT-1 and IR alpha1 were similar to the mRNA expressions.</p><p><b>CONCLUSION</b>Osteoblasts obtained from diabetic rats' mandibles keep the adaptation changes to hyperglycemia and hypoinsulinemia, which may contribute to their dysfunction.</p>

ABSTRACT

PURPOSE: The aim of this article is to analyze the preference for treatment modality of dentists. MATERIAL AND METHODS: Data of 20,038 patients was involved. Data analysis were done by distribution according to the various kinds of prosthesis, including complete denture, removable partial denture, fixed partial denture, implant-supported dental prosthesis as well as distribution according to the professional titles of the dentists finishing the treatment, including resident and professors. RESULTS: The number of cases of dental prosthesis increased year by year. 61.06% of the patients accepted fixed partial denture restoration. The number of patients who accepted implant supported restoration is also increasing year by year. The number of complete denture, implant-supported dental prosthesis finished by professors was larger than that done by residents, while it was contrary for removable partial denture, fixed partial denture, and the difference was statistically significant (P<.05). CONCLUSION: Professors and residents have some difference in the categorization of prosthesis finished. Fixed partial denture and implant-supported dental prosthesis are preferred.

ABSTRACT

BACKGROUND:TiNi shape memory alloy(TiNi-SMA)distractor can be buried completely,there is no time delay and it is different from other distractors previously used.Therefore,it is necessary to reevaluate the osteogenetic time and quality.OBJECTIVE:To investigate the quality of the alveolar ridge augmented by TiNi-SMA distractor by bone mineral density (BMD)and biodynamics tests of new-formed bone.DESIGN:Randomized controlled observation.SETTING:Department of Stomatology of General Hospital of Chinese PLA.MATERIALS:Twelve healthy adult male mongrel canines(ranged from 21 to 26 kg)were provided and bred by Medical Experimental Animal Center of General Hospital of Chinese PLA.All the animals were accorded with the requirements of the quarantine and animal ethnics.Self-made distractors were used in the study.XR-36 Dual Energy X-Ray Absorptiometry (Norland,USA)and 858 Mini Bionix Ⅱ Biomaterial Testing Machine(MTS,USA)were used in the study.METHODS:The experiments were performed at the Medical Experimental Animal Center of General Hospital of Chinese PLA from July 2000 to May 2004.The animals were grouped into group Ⅰ(3 months group)and group Ⅱ(6 months group)randomlv with 6 animals in each group.The left mandible was used as experimental side and the right was used as COntrol side.The canines were kilied after anaesthesia at 3 or 6 months later in both groups.After the mandible samples were cut,the BMD of three points in the distraction gap,above the distraction gap and below the distraction gap were measured by dual energy X-ray.Compressive biomechanics test was performed in the middle of the distraction area of the experimental side and the corresponding area of the control side to detect compressive stress and the modulus of elasticity.MAIN OUTCOME MEASURES:①BMD of the distraction area of the experimental side and the control side in the group Ⅰand group Ⅱ.②Compressive strength and modulus of elasticity of the mandibles of the experimental side and the control side.RESULTS:All of the 12 experimental canines were included in the final analysis,no drop-out.BMD determination:BMD was lower in above the distraction gap of the mandibles of the experimental side than in the control side in the group Ⅰ (t=-2.898,P<0.05).There was no significant difference in BMD between both sides in the group Ⅱ(P>0.05).Compressive biomechanics test:Compressive stress and the modulus of elasticity of the mandibles on the experimental side were lower than on the control side in the group Ⅰ(t=-3.274,-3.534,P<0.05).All indexes increased by some degree in the group Ⅱ compared to the group Ⅰ (P>0.05).CONCLUSION:The new bone is strong enough for later repair such as implantation after distraction osteogenesis using TiNi-SMA distractor.

ABSTRACT

@#ObjectiveTo investigate the feasibility of construction transplantable tissue engineered bone in nude mouse as in vivo bioreactor. MethodsSelected the 4th passage of rabbit bone mesenchymal stem cells, which were cultured, expanded and induced in vitro. Cells were seeded into porous natural coral. The complexes were implanted subcutaneous in nude mice. Regenerated bone was taken out 8 weeks after implantation and implanted into muscle tissue interspace in the back of rabbit itself.The rabbit-anti-mouse antibody titer was measured at 14,28,56 days and the level of IL-2 was detected the at 2, 7, 14, 28 and 56 days after implantation.Immunohistochemistry staining was carried on in the 8th week. ResultsDuring the whole experimental process, IL-2 activity couldn't be measured.The rabbit-anti-mouse antibody titer was positive.A specific antibody reaction was detected from 2ed week to 8th week after transplantation and touches the peak value four weeks later.Immunohistochemistry staining result was negative.ConclusionThe tissue engineered bone growth in nude mouse is seemed to be the reliable transplantable bone source.

ABSTRACT

<p><b>OBJECTIVE</b>To improve the recognization and treatment for oral venous lakes (OVL).</p><p><b>METHODS</b>Clinicopathologically analysis of 20 cases of oral venous lakes was performed. All the samples were removed by surgically.</p><p><b>RESULTS</b>All the lesions occurred in senile persons and located under the mucosa. Among the 20 cases, 11 were male patients and 9 were female; 18 of 20 occurred in the lower lip, another two in the upper lip and buccal mucosa. The lesions were 0.3 approximately 1.2 cm in size. Pathologically, the oral venous lake was composed of single or a few large, dilated and irregular veins. Thrombosis and organization were found within the lesions sometimes.</p><p><b>CONCLUSIONS</b>Oral venous lakes are a kind of focal anomalies of venous structure. All the 20 cases (with a follow-up time from 8 months to 4 years) has no recurrence after surgical treatment.</p>

Objective To observe and analyze the clinical and laboratory features of the use of porcelain fused to metal(PFM) prosthesis in aged patients with loss of teeth.Methods The clinical characteristics of 63 aged patients with 138 units of PFM prosthesis were reviewed and analyzed on the fabrication and reparation processes,and all the patients were followed up for 3 months to 1 year.Results All the PFM prosthesis fit well with the abutments without loosening or drop off except 3 patients,in whom the porcelain cracked.The periodontal tissue remained healthy,and patients were satisfactory with the treatment.The requirements and key points of PFM prosthesis:(1) there must be 6-7 teeth or dental roots left in different sections of the jaw.Bone absorption around the tooth should be less than half of the root.(2) The restoration of the vertical range must be accurate.(3) Adjust the overall relations of the jaw.(4) Eliminate the early contact points to avoid crack of porcelain.(5) Choose the correct temperature and observe strictly operation protocol according to the characters of the porcelain.Conclusion Porcelain fused to metal prosthesis is a good choice in repairing the defect of dental bodies and dentition in aged patients.

ABSTRACT

The aim of the study was to observe osseointegration of dental implants in osteoporotic rats. Rats were randomly divided into groups A(SHAM) and B(OVX), and pure titanium screws were implanted in the distal half of the right femurs. 1 5 and 3 months after implantation, half of the rats in each group were sacrificed and right femurs were observed by X ray photography, scan electron microscopy as well as light microscopy. The results showed that 1.5 and 3 months after implantation, the osseointegration index (OI) of group A was higher than that of group B. But the OI of each group was almost the same at different intervals. 1 5 months after implantation, the amount of new bone formation in group A was more abundant than that in group B, and it increased in group A but decreased in group B 3 months after implantation.The results confirmed that with higher BMD osseointegration was better. Osteoporotic changes could decrease OI and the amount of new bone formation at implant bone interface in rats. Osseointegration would not improve without outside factors.

ABSTRACT

Objective:To evaluate the feasibility of fMRI on the location of brain regions related to cold stimulated toothache. Methods: 8 patients with cold induced dentinalgia of the right maxillary bicuspid and 8 healthy volunteers with full and healthy dentition were selected to perform the fMRI study, all of which are right handed. Block designed blood oxygen level dependent(BOLD) functional MRI scan covering the whole brain was carried out. SPM2 software was adopted to generate the activation map. Results: The patient group showed significant bilateral activations in insula/SII, basal ganglion, cingulate gyrus, midbrain red nucleus. BOLD signals were also found unilaterally in right thalamus, inferior parietal lobule(BA7) and left inferior frontal gyrus(BA44), MII/PMC. In contrast, the normal group showed increased signals in bilateral PMC and inferior parietal lobule BA40. Left lentiform nucleus, SI/MI and right inferior parietal lobule(BA7) were also excited in none painful cold sensation. Conclusions: Motor related cortex including PMC, SMA and BA44 have strong signals during dental pain. Compared with limb pain, tooth pulp stimulation evoked a more pronounced activation in the ipsilateral hemisphere in some brain regions.

ABSTRACT

Objective:To develop an automatic and continuous distraction osteogenesis technique by using totally embedded distractor of nickel titanium (Ni-Ti) shape-memory alloy for the reconstruction of mandibular ramus. Methods:16 adult New Zealand rabbits were used in the experiment. Mandibular ramus and condyle on one side was removed to create a 1.5 cm segmental defect. A transport disc was created by "L" shape osteotomy at the end of the remnant ramus and Ni-Ti distractor was fixed and totally embedded in tissues. The rabbits were sacrificed 2 months after operation and the mandibles were harvested.The bone reconstruction were studied with radiographic and histological examination. Results: 2 months after operation all the animals were alive and mandible function kept well.The Ni-Ti distractors were self-activated and performed distraction osteogenesis automatically. The mandibular ramus achieved preliminary reconstruction. Histological examination showed bone regeneration in the distraction area. Conclusions: Ni-Ti distractor can fulfill automatic and continuous distraction osteogenesis.

ABSTRACT

Objective To study the biosafety of colored alumina-glass composite by mucosa irritation test. Methods The colored alumina-glass composite and a non-irritating material were separately fixed to the oral mucosa of a mouse. The contact parts were examined with bare eye, histologically and cytologically. Results No obvious inflammation of the mucosa was detected in the site where colored alumina-glass composite was in contact. Conclusion Alumina-glass composite has good biosafety.
